Devils Lake Public School District Activities

The Devils Lake School District is proud to offer students a variety of activities to participate in throughout their academic career. Activities include competitive athletics, clubs, organizations, and performing arts.

Physical Form

All students participating in athletics must have a physical examination before they can participate in practice or a contest. We encourage you to schedule physicals early so you do not run into problems with appointments when practice begins. Physical Forms can be printed from the link below or picked up in the Main Office.

NDHSAA Physical Form

The North Dakota High School Activities Association now requires a physical examination every year. Only the final page of the form will be turned in to the Activities office. The physical must be taken on or after April 15th to be valid for the next school year.

Concussion Certification (Submitted After August 1 of Each School Year)

Concussion management is required by North Dakota State Law (NDCC 15.1-18.2). The statute requires that each student and their parent/guardian shall document that they have viewed information regarding concussions that may be incurred by students participating in athletic activities. These forms must be returned to the Activities Office prior to the athlete beginning practice. The concussion forms must be submitted each year that an athlete participates.

Parents/Guardians and student athlete need to sign the permission form. This form must be signed and returned to the school office prior to the athlete beginning practice.

Parent Transportation Form

We would like for all students to ride school transportation to and from all activities. In case an athlete desires to ride transportation other than what is provided by the school district, his/her parents must submit a written request to the coach or administrator. Unless extenuating circumstances prevail, athletes will only be allowed to ride with their parents. All requests to ride with those other than the athlete’s parents must be approved by an administrator before the fact. ALL REQUESTS MUST BE WRITTEN AND WITNESSED BY THE COACH OR AN ADMINISTRATOR. When students do not ride school provided transportation, the school is not liable.
